Provides primary medical care for children and youth such as general physicals, vaccinations, treatment of acute and chronic illness, health education, treatment of ADD/ADHD, anxiety and mild to moderate depression.

Also assists in applying for the Oregon Health Plan and private insurances on the Federal Health Exchange.

INTAKE PROCEDURE: Call to schedule an appointment. Same day appointments available.
FEES: Accepts OHP and some private insurances. Sliding scale fees available for uninsured and underinsured patients. Minimum $20 fee due at time of medical appointment for uninsured patients. No one is refused care due to an inability to pay.
DOCUMENTS: Insurance card, if applicable.
LANGUAGES: English, Spanish, interpretation of other languages by arrangement.
