INTAKE PROCEDURE: Call for information. Must complete pre-intake eligibility and use history screening before admittance.
FEES: Accepts all major insurance and Medicaid. Sliding scale fee for private pay or extremely low-income clients.
DOCUMENTS: None required at intake but would prefer Photo ID and insurance information if availiable.
