Provides a wide range of primary and preventive health care services to the community. Services may include general medical exams, women's annual exams, pediatric care, prenatal care, care for acute and chronic conditions, vaccinations, mental health evaluations and counseling services, and drug and alcohol assessment.

Also provides health education, laboratory services, and prescription assistance services to established clients.

FEES: Accepts Medicare, Medicaid, and most private insurances. Sliding scale fee and payment plans available to individuals with high deductibles or no insurance.
DOCUMENTS: Insurance card, if applicable. Proof of income if needing sliding scale fee.
