INTAKE PROCEDURE: Call for a pre-assessment phone interview and to set up an intake/assessment appointment. Assessment session with a counselor will determine level of care.
FEES: OHP and some other insurance plans accepted. Sliding fee scale for clients paying in cash.
DOCUMENTS: Photo ID. If pregnant, proof of pregnancy.
