INTAKE PROCEDURE: Walk in or call for more information. Clients will get a food bank card once they have verified their address.

DOCUMENTS: Verification of all people in household; one document is required for each member.

Cell phone bill
Oregon drivers license or ID
Oregon Health Plan card or paperwork
DHS household verification print-out
School records or report card indicating the child’s name and address
