HIV/AIDS CASE MANAGEMENT
Provides confidential case management to individuals living with HIV and AIDS. This program provides expense assistance for HIV medications, transportation costs to medical care appointments, insurance premium payments, transitional housing and rent payment assistance, dental care, disease management education, referrals to other agencies for basic needs, employment, and substance abuse treatment.
