TRANSITION SERVICES
Assists individuals of all ages and living with any type of disability with transitioning from parental housing, group home living, nursing homes, or correctional facilities into the community.

Eligibility: Individuals with significant physical, sensory, cognitive, or mental disabilities and their families. Disabilities include:

-Physical or orthopedic disabilities
-Psychiatric disabilities
-Learning or developmental disabilities
-Hyperactivity or concentration difficulties (ADD or ADHD)
-Brain injuries caused by accident or stroke
-Blindness or nearsightedness
-Deafness or being hard of hearing
-Multiple chemical sensitivity (MCS) or environmental illness (EI)
-HIV/AIDS
