PRIMARY MEDICAL CARE SERVICES
Provides patient-centered primary care. Services include care for acute and chronic illnesses, annual exams for children and adults, immunizations, mental health screenings, alcohol and drug assessments, and health education related to patients' needs and issues. Reproductive health services include STI/STD testing and treatment, pregnancy testing, and Pap tests. Also offers assistance applying for Medicaid (OHP) and private insurance through the Federal Health Exchange.

FEES: Accepts OHP (Health Share, FamilyCare, and Open Card), Medicare, and most private insurances. Sliding scale fee available. No insurance, $25 down payment for first visit.
