Oregon's public health nurse home visiting program that works to improve the health outcomes of vulnerable children and pregnant persons through prevention and early identification of problems. The home visiting nurse can provide health and developmental screenings, health and nutrition education, infant and child development tips, breastfeeding support, pregnancy education and referrals to community resources that center on the family's needs.

Eligibility: Families with children ages 0-5 or pregnant people with a higher risk for health issues. Risk factors may include drug-exposed infant, prematurity, teenage parents, at-risk caregiver, domestic violence, and/or concerned parent or provider.
